First Visit Checklist
If you are visiting Dr. Millett's Vail orthopedic practice for the first time, please bring your insurance card or other health coverage information (insurance our office accepts). Some insurance carriers require you to have a referral or primary care physician authorization before a specialty, such as orthopedic treatment, is given. Please check with your carrier if you are in doubt.
For a new patient's initial visit, Dr. Millett will require new, or at least recent, x-rays of the affected extremity. Please be aware that new patients or those who require x-rays may be asked to come in early to obtain these important studies. We recommend patients bring the following items with them on their first visit depending on where their injury is located:
- All old records, x-rays, CT scans, MRI's; always bring the actual films if available
Prior to arriving for your first appointment, it is helpful to have some of the required paperwork completed. Please print and complete the Initial Patient Evaluation Form located on this website and bring it with you.
Payment
Payment in full is expected at the time of your visit. Exceptions include workers compensation, Medicare, and preferred provider contracts (copay only). Check your insurance information to identify whether the Steadman Clinic is a preferred provider for your policy. Charges for physician therapy may also be included in the total charge of your appointment, but are billed separately. For your convenience we accept Visa, MasterCard, American Express, Discover, cash or check. Please contact our office if you have any questions or concerns.
Medical Records
If in the future you need to request certain medical records from our office, please be aware that the request could take ten days to two weeks before you receive the information. We will make every effort to fulfill all medical records requests as quickly as possible.
